Chestnut Health Systems is seeking full-time Case Managers for our
Bloomington, IL facility.

Act as a wellness coach for patients involved in behavioral health
programming, including using motivational interviewing to engage
patients in wellness activities.
Complete evaluations as directed.
Provide education on wellness and treatment services to potential
referral sources and project participants.
Provide pre-engagement sessions to selected clients.
Complete required paperwork on each client, including but not
limited to Chestnut's opening paperwork, evaluations, and progress
notes. Required documentation is to be submitted on the day
services are provided unless otherwise approved by supervisor.
Provide ongoing monitoring of services contained in the
individualized treatment plan to ensure timely provision of

requests for client assistance funds.
Provide ongoing support and advocacy for clients especially around
