Chestnut Health Systems is seeking a full-time Assessment
Clinician to work primary days with some evening hours for our
Bloomington, IL location.

use, mental health and primary care treatment to patients in
Central and Southern Illinois. We provide fully integrated care for

Conduct diagnostic and assessment evaluations with clients and
their families; write bio-psychosocial reports to address diagnosis
and patient placement criteria with structured guidance and
Facilitate groups as required by the program.
Maintain records of clients and all services provided within

as probation, family, psychiatrist).
Provide court testimony when requested.
Attend and participate in all program and staff meetings as

Plan and implement treatment for clients with structured
guidance and oversight.
Conduct admissions and discharges.
Develop Master Treatment Plans (MTP) and determine continuing care
plans with structured guidance and oversight.
Provide individual, family and group counseling and rehabilitation
services to clients and their families (basic level – no dual
diagnosis or complex diagnosis). Clinical functions determined by
individual training, experience and assigned area(s) of
Must have a high school diploma or equivalent and five (5)
years’ supervised mental health or SUD experience; or a bachelor’s
degree in counseling, social work, education, psychology or related
human service field; or be a non-degreed Certified Alcohol and Drug
Abuse Counselor (CADC) in good standing with the State of
Master’s degree in counseling, psychology, social work or related
Must have a valid driver’s license, private automobile insurance
and be insurable (when applicable).
Must obtain CADC within 2.5 years of hire.
Requires basic computer skills and experience to include MS Word,
MS Excel, Outlook, Adobe Acrobat and an electronic medical record
Satisfactory completion of Chestnut’s background checking
