Chestnut Health Systems is seeking a full-time Clinician for our
Supported Housing Program to work primarily daytime hours Monday
through Friday with occasional evening hours at our Bloomington, IL

use, mental health and primary care treatment to patients in

Complete required data entry and paperwork in accordance with
program and agency policies and procedures, including but not
limited to opening requirements, IM-CANS including applicable
modules and other applicable assessments, Health Risk
Assessment/Physical Health Assessment (HRA/PHA), progress notes and
any other assigned requirement: 100% of IM-CANS to be completed
prior to due dates, and Progress notes and any other required data
entry or paperwork submitted within forty-eight hours of the
delivery of the service.
Services primarily to include counseling, community support and
medication training and monitoring; and services to support
systems, including maintaining regular groups and individual
service to consumers.
Services that help maintain a clean environment, including
encouraging, modeling, and at times completing cleanliness tasks,
to ensure a safe, clean environment.
Complete, maintain in working order and submit accurate and timely
documentation, as assigned, to meet housing funder requirements,
including but not limited to, tenant recertification
Conduct and complete initial tenant applicant interviews; oversee
move-ins/move-outs and complete associated documentation; work in
conjunction with facility, purchasing and clinical staff; ensure
all required city and county registrations are submitted.
Must have a Bachelor’s degree and five (5) years’ supervised
mental health experience.
Master’s degree in counseling, psychology, social work or related
Requires basic computer skills and experience to include MS Word
and the use of an electronic medical record (EMR).
Must have a valid driver’s license, private automobile insurance
and be insurable (when applicable).
Satisfactory completion of Chestnut’s background checking
