Chestnut Health Systems is seeking a full-time Case Manager for
our Bloomington, IL facility.
Chestnut Health Systems is a leading provider of behavioral and

use, mental health and primary care treatment to patients in
Central and Southern Illinois. We provide fully integrated care for

Provide complete assessments, including level of functioning of

Provide ongoing monitoring of services contained in the

substance abuse issues are protected.
Provide appropriate mental health/SUD services/interventions,
including crisis intervention, case management, individual and
group counseling, family counseling and/or consultation and/or
client centered consultation.
Document treatment to clients in accordance with established agency
policies and procedures; required paperwork to be completed and
submitted in a timely manner. Required documentation is to be
submitted on the day services are provided unless otherwise
approved by supervisor.
Attend 100% of monthly, required Rule 140 supervision if not an
LPHA and ensure supporting documentation of same.
Attend staff meetings for the Residential Programs.
Attend all mandatory Chestnut trainings and CEU requirements if

Participate in program specific training, educational activities
and intra-agency meetings in order to achieve/maintain professional
development and upgrade knowledge/skills.
Promote the recovery model of treatment including trauma-informed
care, hope, respect, empowerment, health/wellness and

control over their own lives through self-directed recovery;
including utilizing recovery language in all applicable

Must have a high school diploma or equivalent and five (5)
years’ supervised behavioral health experience; or a Bachelor’s
degree in counseling, social work, education, psychology or related
human services field.
Must obtain CADC certification within two (2) years’, if teaching
Must have a valid driver’s license, private automobile insurance
and be insurable if transporting clients.
Requires basic computer skills and experience to include MS Word,
MS Excel, Outlook, Adobe Acrobat and an electronic medical record
Satisfactory completion of Chestnut’s background checking
