Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking full-time or part-time, temporary Pandemic Health Navigators for our Bloomington, IL facility. The anticipated end date of the assignment is December 31, 2021.

We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Build rapport and connection with clients in an empathetic manner.
Assist clients in scheduling, accessing, and utilizing telehealth services with their primary care provider. If the client does not have a PCP, work to get the client established with a PCP.
Screen clients for Social Determinant of Health Issues (SDOH) and make service referrals as appropriate.
Provide clients with resource information by utilizing local database systems and the IPHA Resource List.
Participate in all required staff development activities, including necessary trainings, supervision, and staff meetings.
Complete timely and complete documentation of all activities in the patient record.
Support interventions consistent with the following treatment philosophies: patient centered, trauma informed, recovery oriented, and strength based.
Participate in outreach events in the community to promote awareness.
Participate in program development activities to improve services to patients and community, including making regular recommendations regarding the improvement of service. May include helping develop training protocols for new staff and/or participating in professional audits.
Operate professionally within a multi-disciplinary team setting and provide guidance to less experienced staff/interns; includes consistent modeling of the Mission, Vision, and Values of Chestnut Health Systems.
Function as a team member by providing support and back up to staff as necessary.
Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.


Must have a high school diploma or equivalent and five (5) years’ experience in a case management related position; or a bachelor’s degree in counseling, social work, education, psychology, or in any other helping field or related human service field.
Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
Must have a valid driver’s license, private automobile insurance and be insurable.
Satisfactory completion of Chestnut’s background checking process.
