Chestnut Family Health Center, a division of Chestnut Health
Systems, is seeking full-time or part-time, temporary Pandemic
Health Navigators for our Bloomington, IL facility. The anticipated
end date of the assignment is December 31, 2021.
Chestnut Health Systems is a leading provider of behavioral and
primary health care services. Over four decades, Chestnut has grown
to over 700 committed, compassionate employees providing substance
use, mental health and primary care treatment to patients in
Central and Southern Illinois. We provide fully integrated care for
all of our patients by combining behavioral health care services
with our community-based primary care health centers. Chestnut also
conducts research and training throughout the U.S.A.

Build rapport and connection with clients in an empathetic
Assist clients in scheduling, accessing, and utilizing telehealth
services with their primary care provider. If the client does not
have a PCP, work to get the client established with a PCP.
Screen clients for Social Determinant of Health Issues (SDOH) and
make service referrals as appropriate.
Provide clients with resource information by utilizing local
database systems and the IPHA Resource List.
Participate in all required staff development activities, including
necessary trainings, supervision, and staff meetings.
Complete timely and complete documentation of all activities in the

philosophies: patient centered, trauma informed, recovery oriented,
and strength based.
Participate in outreach events in the community to promote
Participate in program development activities to improve services
to patients and community, including making regular recommendations
regarding the improvement of service. May include helping develop

Must have a high school diploma or equivalent and five (5)
years’ experience in a case management related position; or a
bachelor’s degree in counseling, social work, education,
psychology, or in any other helping field or related human service
Requires basic computer skills and experience to include MS Word,
MS Excel, Outlook, Adobe Acrobat and an electronic medical record
Must have a valid driver’s license, private automobile insurance
and be insurable.
