Job Description:
Overview Under the direction of a Registered Nurse (RN), the
Home Health Aide (HHA) functions as a support to the licensed
nursing staff and performs activities related to the personal care
and hygiene needs of an assigned patient group. Facilitates
communication with other disciplines, patients, family, caregivers,
medical staff and employees. Serves as a public relations liaison
for the unit. Works under the direct supervision of the nursing
manager. Qualifications Certifications:CNA, Education: HS/GED ,
Valid Driver's License, Proof Auto Insurance, and Reliable
Transportation Responsibilities Completes documentation as per
policy and procedure in order to maintain a thorough and accurate
clinical record for each patient.Reports any changes in patient
status to the licensed staff.Practices in a manner sensitive to the
needs of patients and families. Provides care according to plan of
care and orders.Assesses, identifies and reports safety issues for
patient and staff.Participates in monthly Home Health Aide
educational in-services and ensures completion of all mandatory
education.Contributes to the efficacy of the interdisciplinary team
by sharing pertinent information and insights to inform on-going
development of plans of care, participating in problem-solving, and
maintaining current knowledge of individual patient care
plans.Works collaboratively with other members of the HHA team to
maximize efficiencies, meet patient needs, encourage positive
HHA-patient relationships, and promote patient satisfaction.Acts as
mentor to new Home Health Aide employees and contributes to the
orientation and training of staff.Participates in unit PI projects.
Is an active member of unit partnership council, unit committees
and/or projects.Performance demonstrates adequate knowledge of
regulatory & accreditation guidelines state, federal, ACHC. ?Assist
with Scheduling of HHA. Monitors vital signs as directed in plan of
care. Provides hair care including shampooing as directed in plan
of care. Assists patient with bathing and personal care as directed
in plan of care. Attends weekly IDT case conference to provide

The compensation range for this position is $17.33per hour -
$28.07per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate’s
experience, qualifications, location, training, licenses, shifts
