salary: $80,000- $85,000, based on experience and licensure How
you’ll make an impact: Join our team as a Social Services
Coordinator (RN or LPN) and use your nursing expertise in a whole
new way—helping residents not just clinically, but socially,
emotionally, and holistically. You’ll coordinate psychiatric and
psychosocial services, provide advocacy and guidance, support
families through transitions, and help residents maintain
independence and fulfillment. From care plan meetings to meaningful
conversations, you’ll be a key part of residents’ day-to-day
wellness. You’ll partner with an interdisciplinary team to support
behavior management, social engagement, and family communication.
You’ll encourage participation in activities, support group and
individual programs, and help residents complete important
healthcare and legal documents to ensure smooth, safe transitions.
And yes—your documentation skills will shine too, as you record
social service plans, goals, and progress with accuracy and heart.
Schedule: Full-time, Monday–Friday, 8:00am–4:00pm, with 1 weekend
per month on-call. What you bring: - Active LPN or RN license (RN
preferred) - Experience in geriatrics - Ideally 3 years in skilled
nursing or long-term care with strong knowledge of discharge
planning and interdisciplinary care - Excellent communication,
