Overview

Chestnut Health Systems is seeking a full-time Case Manager for our Bloomington, IL facility.



Chestnut Health Systems is a leading provider of behavioral and primary health care services. Over four decades, Chestnut has grown to over 700 committed, compassionate employees providing substance use, mental health and primary care treatment to patients in Central and Southern Illinois. We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Responsibilities

Provide complete assessments, including level of functioning of clients' needs.
Meet with clients and their support networks to develop an individualized treatment plan.
Complete required paperwork on each client, including but not limited to Chestnut's opening paperwork, mental health/SUD assessments, individual treatment plans, and progress notes.
Provide ongoing monitoring of services contained in the individualized treatment plan to ensure timely provision of services.
Provide for referral, linkage and discharge to recommended CHS services and/or community resources, including requests for client assistance funds.
Provide advocacy, both client and system centered, to ensure that basic rights of the chronically mentally ill and those living with substance abuse issues are protected.
Provide appropriate mental health/SUD services/interventions, including crisis intervention, case management, individual and group counseling, family counseling and/or consultation and/or client centered consultation.
Document treatment to clients in accordance with established agency policies and procedures; required paperwork to be completed and submitted in a timely manner. Required documentation is to be submitted on the day services are provided unless otherwise approved by supervisor.
Attend 100% of monthly, required Rule 140 supervision if not an LPHA and ensure supporting documentation of same.
Attend staff meetings for the Residential Programs.
Attend all mandatory Chestnut trainings and CEU requirements if applicable.
Perform on-call duties as assigned.
Participate in program specific training, educational activities and intra-agency meetings in order to achieve/maintain professional development and upgrade knowledge/skills.
Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery; including utilizing recovery language in all applicable documentation.
Promote Chestnut's culture of customer service excellence through the adherence to Chestnut's behavioral standards for customer service.

Qualifications

Must have a high school diploma or equivalent and five (5) years' supervised behavioral health experience; or a Bachelor's degree in counseling, social work, education, psychology or related human services field.
Must obtain CADC certification within two (2) years', if teaching groups.
Must have a valid driver's license, private automobile insurance and be insurable if transporting clients.
Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
Satisfactory completion of Chestnut's background checking process.
